Does anyone have a form for a family limited partnership and is it appropriate to use to avoid liabiliies that, for example, might be incurred by doctors?

The Lipman's Wills and Trusts volume has a form, and yes, limited partnerships are appropriate vehicles for protecting doctors' assets.
