New York Guardianship Volume

The Guardianship volume contains three groups of forms: 1) relating to a guardianship of the person of a minor as established in Family Court, 2) relating to a guardianship of the person and/or property of a minor OR of a mentally retarded or developmentally disabled person in Surrogate's Court, and 3) local forms for Bronx and New York Counties relating to a guardianship of an incapacitated person as established in a Supreme Court.

		Guardianship in the Surrogate Court The volume includes all of the New York State Unified Court System Surrogate Court forms for Guardianship of the person OR the property of a minor or a mentally retarded or developmentally disabled person. Among these forms are the Surrogate's Court forms for the annual accounting of the guardianship, petitions and orders for withdrawal of the property for support and education, and petitions and decrees to close the guardianship.
		Local forms The volume also includes local guardianship forms for Bronx County and New York County.
